Helen Evans Home (ICF/DD) has been serving children and adults with developmental disabilities since 1963. We are a 42-bed residential facility for infants, children and young adults with multiple-disabilities such as intellectual disability, cerebral palsy, epilepsy, and other genetic disorders. Our age range is from 0 to 50. We’ve worked hard to create a home-like environment while providing optimal level of health and maximum personal growth and development.

We provide an interdisciplinary approach to patient care. We put an emphasis on staff training and development to ensure that the residents get the best care possible in a loving and learning environment.

What We're Looking For:

  • We are currently looking for skills LVN's to join our team. Under the supervision of the RN, the LVN is responsible for providing nursing services to clients within scope of practice.
  • What You’ll Do:
  • Administration and documentation of medications and treatments by mouth and/or by g-tube as ordered by physician.
  • Work with Appointment coordinator regarding appointment needs and will ensure that all paperwork and follow up needs are completed after appointments.
  • Communicate with the facility QIDP and professional team of consultants regarding client progress and needs.
  • Must have knowledge of clients’ rights and must protect and advocate for these rights. This includes knowing about mandated reporting of suspected abuse (what to report and to whom).
  • Must attend all required inservice training sessions and staff meetings. A minimum of 3 hours of inservice training per month is required of all direct care staff/attendants.
  • Communicate all health related issues in a timely fashion to the RN consultant and/or Medical Director.
  • May accompany clients to medical appointments as assigned and be able to communicate with the medical professional as to the needs, problems of the client or will follow up to obtain the information as requested by the medical professional at medical appointments.
  • Be able to communicate appropriately with outside agencies and clients’ families.
  • Must have knowledge of how to respond to emergency situations. This includes participation in emergency drills.
  • Monitor and provide proper maintenance for clients’ personal properties, including clothing, toiletries and other personal belongings.
  • Ensure for client safety by maintaining a safe environment and demonstrating safe work habits (proper lifting, proper use of equipment and wheelchair care).
  • Communicate any concerns or needs of the facility or clients to appropriate management staff.
  • Must complete all documentation as required. Documentation includes, but is not limited to charting for Activities of Daily Living, medication administration, incident reports, health progress notes, and behavior data.
  • Perform other duties as assigned.

What You’ll Bring:

  • Graduation from a state-approved school of vocational nursing
  • Passage of the National Council Licensure Examination for Practical (Vocational) Nursing (NCLEX-PN)
  • Active and unrestricted LVN state license
  • 2 years experience as a LVN preferred but not required.
  • American Heart Association (AHA) CPR certification
  • Ability to safely lift or carry 30 pounds
